What are Coughs, Colds & Flu?

Coughs, colds and influenza affect most, if not all, of us at least once a year, but children seem to be more susceptible to them than adults and far more likely to be treated for them when they occur. In the UK & USA, more money is spent On over-the-counter medications for coughs and colds than any other childhood ailment. Yet, while some of these treatments may relieve the symptoms associated with colds and coughs, none will, in fact, effect a cure, all interfere with the body’s own efforts to cure itself (e.g. by ‘drying up mucus and many are associated with dangerous side effects. The truth is that without treatment a common cold will usually last about one week - with or without treatment.




Known Causes

There are two basic theories – the chill theory and the viral theory. The chill theory is based upon the fact that there is a much higher incidence of colds and flu in the winter and that these ailments are related to exposure to the cold weather. This theory, a favourite of mothers and grandmothers, argues that colds are brought about be failing to dress in warm enough clothes - coats. gloves, mittens and hats - during the cold weather.
The viral theory on the other hand suggests that colds are caught more frequently in winter because children are in school, confined in classrooms devoid of fresh air. Both theories may hold some truth; the flu, colds, and coughs are often caused by viruses, but exposure to the cold may lower the body’s resistance to these viruses.



Symptoms

The symptoms of the common cold vary from one person to the next, however, they generally include a watery, irritated eyes, a runny nose, coughing, sneezing and a low-grade fever. Influenza often also includes body aches, vomiting, diarrhoea arid a high fever. If nasal secretion is clear, gray or white, it is usually an indication of a cold or flu, whereas if the secretions are yellow it can indicate the presence of pus, which is an indication of bacterial sinusitis.

Colds can accompany other bacterial infections such as bronchitis or otitis (middle ear infection). Common colds and the flu do not, be themselves, require any invasive medical treatment (as mentioned above). This is also true of viral pneumonia a disease which can only be confirmed by X-ray and. in any event, is associated with mild symptoms. Bacterial pneumonia, on the other hand, is a very serious condition and does require immediate medical attention. The symptoms of bacterial pneumonia include a fever above 102 degrees, severe shortness of breath, aversion to bright lights, stiff neck and blueness of the skin.



Conventional Treatment

The medications most commonly used for treatment of cold and influenza symptoms fall into six categories: decongestants, expectorants, antihistamines, cough suppressants, pain killers and, last but not least, antibiotics. According to Dr Robert S Mendelssohn MD, who was a pediatrician for over thirty years and former chairman of the Medical Licensing Committee for the State of Illinois and associate professor of Preventive Medicine & Community Health in the School of Medicine of the University of Illinois all of these medications have a number of things in common - they are unnecessary they are associated with several harmful side effects, they interfere with the body’s own efforts to defeat the disease, and they are a waste of money!

What is Cataract ?
A cataract is the condition in which the crystalline lens of the eye becomes clouded or opaque, impairing vision and, if untreated, can lead to blindness. Nearly two-thirds of all cases in Western countries affects people over the age of 65.

Known causes
The lens in the eye transmits light rays onto the retina at the back of the eyeball which are then interpreted by the brain into images. It is also through the lens that we are able to focus on objects, whether they be a few inches away or 100 yards in the distance. A tiny muscle in the eye, known as the ciliary muscle, changes the shape of the lens to enable the eye to focus on different objects. If the lens becomes opaque or clouded, its shape cannot be effectively controlled by the ciliary muscle which prevents proper focussing on objects and, due to the clouding of the lens, general vision becomes impaired and can even lead to blindness.

There are thought to be many potential causes of cataract:- The vast majority of cataracts are considered to be the result of an age-related change in the lens unassociated with other disease processes. As we age, the structure and function of the lens begins to degenerate. Through a complex series of chemical events, opacification ensues.

• Cataracts may be secondary to another eye disease. Intraocular inflammation, retinitis pigmentosa, glaucoma, and many others can be associated with lens opacity.
• Cataracts may be toxic, i.e. drug-induced or diet-induced. Cortisone-type steroids are the most commonly implicated medications.
• Cataract may be caused as a result of injury in early life. (Rapidly forming cataracts usually occurs after direct injury to the lens from an object that penetrates or cuts open the eye.)
• Systemic illnesses have also been associated with cataract formation. Diabetes mellitus may be the most common, but the list includes Down's syndrome, myotonic dystrophy, Marfan's syndrome, allergic dermatitis, and many other diseases.
• Congenital or infantile cataracts can occur spontaneously as a primary disorder, or secondary to systemic diseases. Among the associated conditions are maternal infection with rubella (German measles) or other agents, metabolic disturbances, hereditary disorders, neurologic diseases, and rarely, birth injury.

Symptoms
The symptoms usually begin with a gradual diminution or blurring of vision. Glare from sunlight or nighttime driving can be a significant problem.

Cataract most often affects people betwen the ages of 50 and 70 but the symptoms may first appear at a much younger age, especially if cataract- associated diseases are present. Congenital cataracts may be dense enough to preclude useful vision beginning at birth.

Conventional Treatment
Conventional treatment for cataracts is entirely surgical: the crystalline lens is removed. There are no known effective medications.

Surgery is usually only considered when the cataract is dense enough to alter the person's ability to perform daily activities, but each individual case is assessed on the specific circumstances. For example, a 55 year old taxi driver may need an operation much sooner than an elderly, bed-ridden patient.

Cataract surgery is available to all individuals in the UK and many thousands of operations are performed each year. In the USA over 500,000 cataract operations are performed each year at a cost of over $3.8 billion. A variety of surgical procedures are performed but the most common method is now the implanting of another lens in the eye to replace the damaged one. The problem, of course, with this treatment is that it does not affect the underlying causes that led to the cataract in the first place. Furthermore, recent research (2) suggests that mental complications can arise as a result of the treatment including confusion, agitation, schizophrenia, aberant behaviour and nervousness which have, in some cases, necessitated sedative drugs and/or psychotherapy.

Aromatherapy - essential oils & Bronchitis

Herbal expectorants are traditionally used for clearing the respiratory tract with non-serious acute and chronic infections of the upper respiratory tract and with obstructive bronchial diseases. They are intended to counteract the accumulation of mucus and the risk of a consecutive bacterial infection. Recently, scientists have discovered that some essential oils can produce the same results as secretolytic and secretomotor drugs. They cause an increase in serous mucous secretion, improve mucous discharge and, more importantly, the oils have antimicrobial and slightly broncho-spasmolytic properties. In one study involving 149 patients with acute bronchitis, influenza infection and rhinitis, an ointment containing eucalyptus oil and menthol was found to be effective at clearing the respiratory tract and reducing inflammation when used for inhalation and embrocation,

  Traditional Chinese Medicine (Massage therapy) used to help treat respiratory tract infection of children


Researchers at the Jianye district TCM Hospital, Nanjing conducted a randomised controlled clinical trial to determine the efficacy of massage, according to Traditional Chinese Medicine (TCM) principles, in the treatment and prevention of recurrent respiratory tract infection of children.
Incorporated into the massage was the principle of "eliminating the pathogenic factors by supporting the healthy flow of energy"
The results showed that the therapeutic effect of the treatment group was significantly better than those not given the treatment. All immunologic indexes were approximately normal when the children were re-examined 3 and 6 months following the massage. The differences were found to be highly significant.
This showed that the Chinese Massage was very helpful in improving the general constitution, enhancing immune functions and preventing and treating the condition.

Breast Cancer

Symptoms

A lump or thickening in the breast or armpit, any flattening or indentation of the breast skin, any puckering, pitting or dimpling of the breast skin, any change in the position, size or colour of the breast, clear or bloody nipple discharge, a retracted nipple, redness of the breast skin are all symptoms.
 


Conventional Treatment

For localized tumors, mastectomy (removal of the whole breast) may be appropriate, but breast-conserving surgery (removal of the tumor and some surrounding tissue, sometimes called lumpectomy) followed by local radiation is often preferable. Although recurrences are more common with breast-conserving surgery, these can be treated by mastectomy, and the survival rates are equivalent to those when mastectomy is used initially. Either procedure may be followed by additional chemotherapy or hormone-blocking therapy. If tumor cells have high levels of receptors for the hormones estrogen and progesterone, it is a good sign because hormone-blocking therapy may stop their growth.
Under study: High-dose chemotherapy followed by reconstitution of damaged bone marrow; chemotherapy before surgery; immunotherapy, including immunotoxins (molecules that combine a toxic agent with an antibody that binds to tumor cells); new chemotherapies and drug combinations. Tamoxifen, a drug that suppresses the effects of estrogen, may help prevent breast cancer in some women at high risk.

What is Back Pain?

Most back pain is caused by muscle strain, trauma or spinal deformity. Only about 10 percent of back pain is caused by a systemic illness. Back pain can develop anywhere from the neck to the lower spine. The pain can be localized or spread across a wide area and radiate from a central point.

Known Causes

Back pain has many causes, including overuse, trauma, degeneration of vertebrae, infection, or tumor. The exact cause of pain may be difficult to identify, since it can come from soft tissue, bone, disc or nerves. Risk factors for low back pain include cigarette smoking, jobs that require repetitive or heavy lifting, and exposure to vibration produced by vehicles or industrial machinery. Certain sports, such as cross-country skiing, and prolonged vehicle driving are also associated with back pain. Diseases such as spinal osteoarthritis, spondylitis and compression fractures can also cause pain. Some of these diseases are more prevalent in the elderly, consequently older people are at higher risk for back pain.
Health Impact
  • Back pain is the most prevalent medical disorder in industrialized societies.
  • Low back pain disables 5.4 million Americans and costs at least $16 billion each year.
  • Two-thirds of all adults will experience at least one episode of back pain in their lifetime.
  • Seventy percent of people with back pain recover within a month. Symptoms persist for more than six months in only 4 percent of the cases, but this group accounts for 85 percent of the money spent on treatment and compensation for low back pain.
  • About half of the people with chronic back pain return to work.

Diagnosis

Physicians should determine whether pain is musculoskeletal, neurological or from one of the organs. This diagnosis is based on a careful history and physical examination. Injection of a local anesthetic and/or steroid into soft tissue or joint spaces can be helpful in diagnosing and treating back pain. Imaging procedures that assist in identifying the pain source include the x-ray, bone scan, computerized tomography (CAT scan) and magnetic resonance imaging (MRI). Expensive imaging procedures are generally reserved for patients whose diagnosis is not apparent with more conventional diagnostic techniques.

Conventional treatments

Over-the-counter pain relievers, such as aspirin, acetaminophen and non-steroidal anti-inflammatory drugs, are often the only treatment necessary for back pain. Patients should avoid any activity that increases the pain. For persistent pain, assistance from a rheumatologist should be obtained. Treatment should be directed at the specific cause of pain. Management techniques include analgesics, anti-inflammatories, antidepressants, muscle relaxants and rehabilitation exercises. Mechanical back supports are usually recommended for limited periods of time in certain situations, such as post-operative patients. If these techniques fail within a reasonable time, injections with local anesthetics and steroids can be helpful. Surgery can be very helpful when comprehensive nonoperative therapy fails. Physical and occupational therapy are important forms of treatment.

What is Appendicitis?
This is an inflammation of the appendix. It is usually seen in young people. The infection may result in swelling, ischaemia, gangrene and perforation.
Rupture of the appendix develops into peritonitis.
What are the symptoms?
The symptoms are slightly different to those of peritonitis. Typically, appendicitis begins with referred pain in the centre of the abdomen spreading to the lower right side of the body several hours later. This is followed by loss of appetite, nausea, malaise, vomiting and unpleasant breath.
Upon examination, there is tenderness, rigidity and rebound tenderness.
Treatment
Early appendectomy, (removal of the appendix is recommended).

What is a Stroke?

A stroke ( otherwise known as a brain haemorrhage or apoplexy) is the term used to describe bleeding in the brain. It occurs when a blood vessel bursts or falters and it can be caused by a variety of factors including physical injury to the head, arteriosclerosis (hardening of the arteries), atherosclerosis (excess cholesterol in the blood) and high blood pressure.
It is also part and parcel of coronary heart disease (CHD) - the most common cause of death in the western world and said to account for nearly 50% of all deaths in the U.K. Contrary to popular belief arterial degeneration does not only affect elderly patients. Of 3,000 US soldiers under 30 years of age who were killed in the Korean war, over 50% showed damage to their coronary arteries and a further 25% showed 'marked changes'.



Known Causes

There are several accepted causes for CHD including:-

1. Diet
2. Obesity
3. Lack of exercise
4. Cigarette smoking
5. Stress
6. Inherited factors
7. Associated diseases - eg. diabetes, gout, high blood pressure
8. The Pill
9. Aspirin *
10. Fish oils **

* Aspirin has been shown to help thin the blood and thereby reduce pressure on the heart and helping to prevent heart disease, BUT at the same time, it also increases the likelihood of arterial stroke.
** Fish oils are also known to thin the blood. In a recent study with adolescents, over 80% of the group taking fish oil supplements had nose bleeds while taking the oil(1) . Furthermore, it is interesting to note that the same research supposed to demonstrate that fish oil lowers the incidence of heart disease in Eskimos, revealed that Eskimos were in fact dying in greater numbers from cerebrovascular haemorrhages (strokes). (2)



Conventional Treatment

In conventional medicine, stroke patients are usually treated with a variety of therapies including medications, surgical implants (shunts) to relieve pressure and physiotherapy. On average, 75% of survivors will walk after one month and 60% will be able to care for themselves with supervision or independently after 3 months. Depending upon the area of the brain affected, the stroke can cause difficulty in speaking, eating, continence, sexual function and mobility. The recovery rate is also influenced by the general health and age of the patient.

What is anxiety?
Anxiety is one of our most common modern feelings. Fear evolved to help us survive so that we could respond quickly if in danger. The fight or flight response was intended to prepare an individual in a dangerous situation to fight or run away. Modern life has unprecedented dangers, but they’re usually not the sort you can hit on the nose or run away from. So our anxiety accumulates and our bodies are powerless to resist its pernicious effects.
What are some of the common symptoms of anxiety?
Anxiety is a diffuse condition that ranges from mild sensations of uneasiness and tension to full-blown, disabling panic attacks. When the latter occur for the first time, it can be a terrifying experience because of the intensity of the symptoms involved.
Temporary anxiety is often the response to a stressful or demanding event, such as sitting an examination or speaking in public. Common symptoms of this form of anxiety may involve an uneasy situation of ‘butterflies’ in the stomach accompanied by muscular weakness.
However, this is generally a much less intense experience of anxiety than the symptoms experienced by those with long-term chronic anxiety. Long-term anxiety leads to persistent problems such as tension headaches, irritable bowel syndrome, insomnia, general fatigue and burn out.
Things that can help alleviate anxiety
Positive life style changes may help, especially in short-term anxiety, as well as obtaining additional help from a counsellor or psychotherapist. Breathing deeply also helps as this makes the body release endorphins, which are hormones having a calming effect on the body. Other therapies and treatments that can help reduce anxiety include aromatherapy, massage and herbs, in particular Kava Kava and Valerian.

What is Anaemia?

What is Anaemia?

Anaemia means that either your red blood cells or the amount of haemoglobin (oxygen-carrying protein) in your red blood cells is low. Haemoglobin is a protein that carries oxygen in your red blood cells.

Iron-deficiency anaemia is the most common form of anaemia. In the United States, 20 percent of all women of childbearing age have iron-deficiency anaemia (compared to 2 percent of adult men). The primary cause is blood lost during menstruation. But eating too few iron-rich foods or not absorbing enough iron can make the problem worse. Pregnancy, breast-feeding a baby, and blood loss from the gastrointestinal tract (either due to ulcers or cancer) can also deplete iron stores. Anyone, older or younger, who has a poor diet is at risk for iron-deficiency anaemia.
Folic-acid-deficiency anaemia is another type of anaemia. It occurs when folic-acid levels are low. Not enough folic acid in the diet and/or poor absorption leads to this type. The need for this vitamin more than doubles during pregnancy. Folic acid may prevent certain birth defects such as spina bifida if taken before conception and in the early months of pregnancy.
Other less-common forms of anaemia include pernicious anaemia (inability of the body to properly absorb vitamin B-12), sickle-cell anaemia (an inherited disorder) and thalassemia anaemia (also inherited).
Alcohol, certain drugs and some chronic diseases can also cause anaemia.



Symptoms
Symptoms of anaemia are tiredness, weakness, and paleness. Paleness could be pale skin. It could also be paleness around the gums, nailbeds or the linings of the lower eyelids.

  • blood in your stools or urine or have black, tar like stools with these problems:
  • Light-headedness

  • dizzy when you stand up or when you exert yourself

  • Do you have menstrual bleeding between periods?
  • Has menstrual bleeding been heavy for several months?
  • Do you normally bleed seven days or more every month?
  • Do you suspect that you are pregnant?

Allergies

What are Allergies?

The term allergy is used to describe a response, within the body, to a substance, which is not necessarily harmful in itself, but results in an immune response and a reaction that causes symptoms and disease in a predisposed person, which in turn can cause inconvenience, or a great deal of misery. Allergy is extremely widespread and affects approximately one in four of the population in the UK at some time in their life. Each year the numbers are increasing by 5% with as many as half of all sufferers being children.



What Is An Allergen?

Almost anything can be an allergen; the most common being house dust mites, pollen from trees and grasses, cats, dogs, insects such as wasps and bees, milk, eggs, peanuts. Less common allergens include nuts, fruit and latex. Protein is often regarded as just something that we eat. It is, in fact, an organic compound containing hydrogen, oxygen and nitrogen, which form an important part of living organisms. There are, however, some non-protein allergens which include penicillin and some other drugs. For these to cause an allergic response they need to be bound to a protein once they are in the body.

What Allergy tests are available?
The type of test to be carried out will depend upon your symptoms or condition of your skin and are described as follows.
Skin Prick Testing:
This is a test, which measures specific IgE attached to cells in the skin important in allergies called "mast" cells. This is probably the most commonly used allergy test and is appropriate for inhaled and ingested (eaten) allergies.
  • It is usually carried out on the inner forearm (palm uppermost)
  • If the patient has bad eczema on the area under test then the test can be performed on the back
  • Ideally the allergens to be selected should be in accordance with the patient’s history
  • As few as 3 or 4 or up to about 25 allergens can be tested
  • The arm is coded with a marker pen for the allergens to be tested
  • A drop of the allergen (extract) solution is placed by each code
  • The skin is then pricked through the drop using the tip of a lancet -this can feel a little uncomfortable but should not be painful
Positive reaction: 
The skin becomes itchy within a few minutes and then becomes red and swollen with a "weal" in the centre (very much like the reaction to a nettle sting). The weal has a raised edge, which slowly expands to reach its maximum size in about 15-20 minutes, clearing for most people within an hour. The size of the weal varies with the average being 3-5 mm in diameter.

Also included in skin testing is a negative and positive control:
The negative control is a saline (salt-water) solution, to which a response is not expected. If however a patient reacts to a negative control, then this will indicate that the skin is, for what ever reason, extremely sensitive and that the results from the allergen challenge needs to be interpreted with the utmost care.

The positive control solution contains histamine, to which everyone is expected to react. Failure to do so could mean that medicines the sufferer is taking could block the response to the histamine and allergens. Patients will be asked to avoid taking anti histamines, cough medicine and some anti depressants (Tricyclic) for about 5-6 days prior to the test.
The skin prick test introduces such a tiny amount of allergen into the skin that testing is quite safe and almost any age group can be tested. However where there has been a clear anaphylactic (shock) reaction to a specific allergen ingested then skin testing may not be appropriate. These tests can be carried out on all age groups including babies although the response will be considerably smaller than in an adult.
Skin prick testing is usually the first test recommended when an allergy is suspected. The advantages are that it is a simple, quick and inexpensive form of testing. It can give useful information in all forms of allergy and provides results within 15-20 minutes. This can be carried out within a hospital or GP surgery environment, by specially trained nurses or doctors.
Blood Test:
The test is carried out on a small sample of blood, which is usually taken from a vein in the arm, using a fine needle and a small syringe, causing minimal discomfort. The sample is then sent to a hospital laboratory and the results are available in 7 to 14 days. The blood sample can be taken at the GP surgery or at a hospital.

These tests are particularly useful when:
  • The patient has a risk of an anaphalactic (shock) reaction, which would make skin prick testing too risky.
  • When extensive eczema makes skin prick testing impractical.
  • When antihistamine medication cannot be stopped because of severe symptoms.
  • No skin prick testing facilities are available
Where unusual and rare allergens are suspected, as there are a wide range of UniCAP® allergens now available for testing
Positive Test: - UniCAP is reported in classes of 0-6 where increasing classes indicate increasing sensitivity to an allergen:

Class 0 Negative
1 Low
2 Moderate
3 High
4 –6 Extremely High

Patch Testing
This test is performed in cases of contact dermatitis (Eczema) where allergy is suspected.

  • The allergens are prepared in appropriate concentrations in white soft paraffin (e.g. Vaseline) and are then spread on to discs, 1 cm diameter.
  • The discs (which are made of a special metal, cannot themselves provoke a reaction) are placed on the skin, usually on the back, and are kept in place by hypoallergenic tape.

  • The skin is coded appropriately and the patient is asked to keep the skin dry. 
  • The patches are left in place for 48 hours, after which time the discs are removed, the skin is examined and any redness or swellings are noted. The skin is re-examined after a further 48 hours for any remaining local redness or swelling.
The interpretation of this form of testing is not as simple as it sounds and tends to be carried out by dermatology departments in hospitals. The symptoms of contact dermatitis need to be brought under control before patch testing can be carried out; otherwise the results will be unreliable. Steroid creams need to be stopped for 3-4 weeks before testing as they may suppress the test response.
Any professional interpreting skin, blood or patch tests must first interpret the results in the light of the patient’s history. No test should be read in isolation.
